PatientPack enables patients to book appointments across their registered GP practice, associated federated/hub practices and outpatient services. At set-up and as part of the localisation process, we identify and map individual roles at a practice or ‘hub’ level so that a patient can be signposted to the most appropriate clinician based on their needs, whether that’s a GP, nurse, healthcare assistant or pharmacist, amongst others.
Patients can view current, past, repeat or acute medication and order prescriptions with all actions captured and recorded back into the patient primary care record. With the Gold package, you can access the functionality to route orders from all patients in practice networks to centralised hubs if required.
PatientPack’s video consultation technology can help to reduce non-urgent GP appointments. Video consultations are simple to organise. Practices can use the technology to perform both single and multi-disciplinary team video calls with patients.
The solution allows unique read and write access to patient medical records without the need for workflow or staff interaction, including cross-organisational systems. Patients can view their health conditions with signposting to content for additional guidance and self-help to pro-actively manage their health. Patients can also see their allergy and immunisation information, and access test results.
The in-built symptom checker uses a unique clinical algorithm to take patients through a set of questions to identify the issue and signpost them efficiently to the most appropriate next step. That could be guiding them to call 111 or 999, signposting them to other, more appropriate healthcare providers or guiding them to book an appointment at the practice. It can also refer them to appropriate self-help information if no appointment is needed. The symptom checker is fully clinically verified and recognises up to 1,200 presenting complaints, including areas such as mental health.
Through the app, patients can monitor and track key health and lifestyle metrics, such as weight, BMI, BP and exercise. It supports effective self-management by providing. Our intelligent technology uses information on patient records to automatically generate a feed of content specific to the patient’s health conditions. It also automatically generates a feed of information related to any current and recent medication and signposts to specific public health or screening services if they fit the required health criteria. Providing personalised self-care information can support patients to pro-actively manage their health and promotes positive health and wellbeing.
Our Target Messaging solution comes as part of PatientPack suite of products and helps GP Practices engage with their patients via batch/bulk SMS text, email or push notifications which are written back to the patient record. Mass text messaging campaigns are a low cost and low effort way for GPs to keep patients updated, particularly for large scale communications such as flu jab reminders. The technology can also be used to send appointment reminders and to target specific patient cohorts by demographics, helping practices to deliver powerful campaigns and see instant results.
Our digital assessments remove the need for physical or telephone appointments with GPs. Patients complete an online review of their condition before sending through to their GP. Assessments are quickly and easily to send out to groups of patients with a specific health condition (e.g. asthma) cutting vast amounts of time. A library of templates is available or assessments can be customised to meet requirements.
Significantly reduce practice workloads by enabling patients to self-refer to any services via the app. Some of our existing practices have set up forms that provide their patients with the ability to refer into maternity services, speech and language, as well as ear care and physiotherapy services.
Our service finder tool enables patients to search for local services to fit their needs. Practice staff will also find this facility useful to guide patients to these additional services and can utilise PatientPack’s patient messaging technology to send them links to the information quickly and simply. Service listings include sources from national NHS databases as well as local services.
A convenient and efficient way for practitioners and patients to correspond. Practitioners can create bespoke messages from scratch or choose from pre-defined templates, speeding up the process for sending standard messages such as clear blood test results, where no further action is required.
Send digital documents (such as fit notes) and images to patients via SMS text message and save time and money on admin and mailing costs. Message exchanges can be saved direct to the patient’s medical record.
Take the pressure off administrative teams, free up telephone lines and drive efficiencies by digitally navigating patients to the most appropriate service for their needs. Using our online triage, patients can select to submit an admin query, check their symptoms, seek online advice and guidance or self-refer into a service via simple online forms which can be set up and configured to meet each individual practices’ needs.
A simple but really effective feature of our PatientPack app is the ability for patients to send through administration requests, simply and quickly without picking up the phone or attending the practice. Patients can use this feature to inform the practice of updated contact details, request a sick/fit note, query their medication, follow-up on a referral, send a general message to the practice/GP, or send a request to join a patient participation group. These patient requests can be set-up to be routed into practice specific workflows by email, the clinical system or our app.

Ideal functionality for practice networks with centralised medication hubs, this functionality allows all medication requests made through the app from a particular group of practices to be routed through to the central hub for processing.
Ideal for mass vaccination programmes and developed in partnership with one of our CCG clients, our vaccination app integrates securely with clinical system providers (EMIS Web/SystmOne), enabling practices to complete the administration process for vaccination appointments in seconds, and off-site.
Our digital care plans enable practitioners to proactively manage and monitor patients remotely, cutting the need for unnecessary appointments. Practitioners can create bespoke care plans based on a patient’s specific health needs, or use our library of care plan templates to ensure quality and standards of care across the patient population. Our care plans incorporate patient goals, so progress can be tracked against them, and prompts can be set up to perform specific assessments and review remote monitoring readings.
Patients with a digital care plan can take readings from devices whilst at home and upload for clinicians to review. When new patient readings are received, they are added to GP workflows via our Desktop app or a clinical system. All readings are automatically coded to medical records and abnormal readings are automatically flagged, ensuring a prompt follow-up.
Allow clinicians and health workers to access your patient records without the need for expensive clinical systems. Using GP Lite, you can provide access to the whole patient record or just specific/relevant parts to enable other GPs, health care workers or out of area A&E departments to view any relevant information, perform a consultation and provide notes on the patient record for the GP to review if necessary. Practitioners can gain patient consent by requesting via SMS or the patient app. Consultation outcomes can be shared with a range of organisations using email ITK messaging or via our APIs.
